National ID (KTP) *
Official government-issued identity card that serves as proof of identity and residence.
Proof of Financial (DTV) *
Attach your bank statement for the last 3 months, showing a minimum average required balance of THB 500,000 or IDR 250,000,000.
Proof of relationship to the DTV visa holder *
Attach the proof of relationship to the DTV visa holder, can be proven with - Marriage certificate - Birth certificate - Certificate of adoption - Family certificate (KK)
Flight booking *
Please provide your flight booking details, including the flight number for your arrival and departure, or you can purchase a flight booking service here.
Hotel booking *
Provide booking voucher of your hotel accommodation or you can purchase a hotel booking voucher here.
Family certificate *
Certificate that shows that you & your dependent are family from your country
