Description
United States B1/B2 Business and Tourist Visa allows its holder to visit the United States for tourism, business, or medical treatment purposes. This visa is valid for up to 5 years, with a maximum stay of 6 months per visit. After completion, you can collect your visa and passport at the SPUN Operations Office, or opt for delivery add-on during checkout.

- 1. No USA visa rejection in the past six months.
- 2. Passport validity of at least three months beyond the planned departure date from the USA.
- 3. Passport must contain at least two blank visa pages.
- 4. Passport must not have been issued more than ten years ago.
- 5. No history of overstaying in any country.
- 6. No criminal record.
- 7. Sufficient funds to support the stay must be demonstrated.
- 8. No history of illegal work on a previous visa.
- 9. No record of deportation from any country.
- 10. Consistency in the stated purpose of travel is required.
- 11. Do not apply for a visitor visa while intending to work.

Provide us your Certificate of Witholding Tax Income (Surat Pemberitahuan Tahunan Pajak). Make sure the tax return is signed in the signature column.

National ID (KTP) *
Official government-issued identity card that serves as proof of identity and residence.
Family certificate *
Certificate that shows that you & your dependent are family from your country
Birth certificate *
Birth certificate showing full name, date, and place of birth.
Passport endorsement page *
Attach the endorsement pages even if they contain no remarks. - For e-passport, this refers to pages 4 and 5. - For polycarbonate passport, this refers to pages 2 and 3.
Colored photo - 5 cm x 5 cm *
- New photos, no older than six months; never been used in any previous visas - Plain white photo background - No white shirt - Photo size 2x2 inches (5x5 cm) or 600x600 to 1200x1200 pixels in digital version; - Faces take up 70% of the photo - Eye glasses are not allowed to be worn for US Visa photos - No edit allowed
Bank statements of 3 months *
Bank statement for the last 3 months. It's advisable to have a stable bank statement with no sudden fluctuations in funds or balance to avoid raising suspicion at the embassy.
Bank reference *
Bank reference letter issued by the bank with a wet stamp and the bank’s contact number. The letter must state the current balance in your account.

Letter of Attorney for USA Visa *
